My Plan Facts reproduces Form 5500 figures without editorial change, so most differences between a page here and a sponsor's own understanding of its plan come from the filing itself.

Errors in the filing

A figure that was entered wrongly on Form 5500 can only be changed by the plan administrator filing an amended return through EFAST2. The Department of Labor's research files pick up amended returns in their monthly refresh, and the amended figures replace the old ones here at the next daily build after that. This site cannot alter a filed record, and it does not judge between a sponsor's account and the filing.

Errors introduced by this site

If a figure here does not match the filed form (a plan attached to the wrong employer, a provider grouped under the wrong name, a mis-computed median, a history joined to the wrong plan), write to the address on the contact page with the page URL and the value on the filing. Processing errors are fixed in the code and the site is rebuilt. Expect a reply within five working days.

Names of people

The site publishes organisations only. Names are screened when the files are read: sponsors, providers and accountants whose name reads as an individual are dropped, and administrator and signer fields are never read at all. If a person's name appears anywhere, report the URL; it is removed immediately and the screen is tightened so it does not return.

Requests to remove a plan

Form 5500 is a public record by statute, and plans of organisations are not removed on request. A plan whose sponsor is in fact an individual is removed immediately on notice.
